Midler is the user of the shapeshifting Stand High Priestess, which has hard teeth which it can use to crush things easily. Initially, the wielder of the Stand was unknown, High Priestess being soundly defeated by Jotaro in Stardust Crusaders, leaving only an unconscious body for the Crusaders to find afterward.

Her current design and name (a nod to singer Bette Midler) first appeared in the CPS-3 game of the series produced by CAPCOM. Upon being asked, Hirohiko Araki created her character to complement the game's roster at that time.

She has enjoyed a degree of popularity since her introduction.
