A meme originating from the Polish Internet. It's a reference to Pope John Paul II's time of death (21:37 on April 2, 2005).

At first, it was used to mock the Pope in the context of pedophilia in churches and the extent of his idolization, but nowadays, it has mostly lost its negative connotation.
